| Please only fill out the contract if you have spoken with an Impact Sound Entertainment representative and have agreed on a price. By clicking the submit button you agree to the following terms and conditions. The client agrees to provide adequate safety, lighting, electrical, and other facility requirements. Client will be held responsible for any damages to equipment, unless caused by entertainer. Client agrees to idemnify and hold harmless, Impact Sound Entertainment, for any claims or causes of action, which may arise relative to this agreement, and the services herein. A non-refundable $50.00 deposit will be required for all shows, if the $50.00 deposit is not received within 2 weeks, contract will be considered null & void unless written approval is received from Impact Sound Entertainment. There shall be no cancellation of this confirmation and binding contract by either party, except upon, at least an (10) ten-week notice, in writing. If client should cancel inside of 10 weeks of event, it must be in writing, and a non refundable reservation fee of $100.00 may be charged in addition to the deposit, but may be applied in reservation for further available date. If client should cancel within (2) two-weeks of event, client event is a considered a no show, or client fails to notify Impact Sound Entertainment of cancellation, total cost is due in full with no discount applied. Impact Sound Entertainment reserves the right to fulfill service of entertainment with person(s) of reasonable qualification and experience.
